Country Gardens is a warm and comfortable 16-bed assisted living community in beautiful Fallbrook, CA. We provide assisted living in a nurturing, home-like atmosphere — specially trained for the care of seniors with memory impairment, including Alzheimer's disease and dementia-related behavioral concerns.
What we do
We specialize in caring for residents with Alzheimer's disease, other forms of dementia, and the social and behavioral challenges that come with them.
Daily visits from multi-disciplinary nurses to monitor blood pressure, blood sugar, and other vitals — coordinated with each resident's physicians.
We work hand-in-hand with trusted local hospice providers to help families and residents navigate end-of-life care with dignity and comfort.
A high staff-to-resident ratio means personal attention. Each resident receives the care, dignity, and connection they deserve — and families get the transparency and peace of mind they need.
